This Form can be completed directly from our website on the Volunteer page. If you are interested in becoming a volunteer, please complete this form and our Volunteer Coordinator, Mary, will contact you to set up volunteer services at The Phoenyx Farm & Sanctuary.
This Form can be completed and printed when you have spoken to the Volunteer Coordinator, Mary, and have a set plan to begin volunteer services at The Phoenyx Farm & Sanctuary.
Note: You can electronically sign, however, it must also be signed in person with a witness from Mary (Volunteer Coordinator) or another staff member that was designated to sign off.
This form/contract is used when you are looking to adopt one of our residents.
